App account and data requests

What the current mobile app action does on this device and how to contact the practice for an account, privacy, or medical-record request.

What the current app action does

The current store build uses a mock patient sign-in. Using Delete app account clears the app's session, stored trusted-device credential and profile, and session-scoped local caches on this device, then signs you out. It does not send a server deletion request, invalidate sign-in credentials, delete a server-side app or portal account, or delete medical records held by the practice. The same mock sign-in information can be used again.

Effective date: August 1, 2026.

Use the current in-app action

  1. Sign in to the Julep Health patient app.
  2. Open Account.
  3. Select Delete app account, then Review and delete app account.
  4. Read the app-account and medical-record retention notice, then select I understand — continue.
  5. Type DELETE in the confirmation field.
  6. Select Delete app account to clear the local app state and sign out.

No server request occurs in the currently configured mock build. This local action is not deletion of an account or information held by the medical practice. To request a server-side account change or make a privacy or records request, use the contact route below.

What is cleared locally

The current action clears the app session, locally stored trusted-device credential and profile, and session-scoped app caches on the device. It then returns the app to its signed-out state. It does not prevent another mock sign-in.

What is unchanged

Because the current action makes no server request, it does not change Julep Health's practice or portal identity record or information that is part of the medical record. Medical records may include clinical messages, documents, photographs, insurance information, appointment information, results, care plans, and other information used to provide or document care.

Julep Health retains medical records and related information for the periods required by applicable federal and Mississippi law, professional obligations, payer requirements, and other record-retention rules. Retained records remain protected under HIPAA and Julep Health's Notice of Privacy Practices. Their availability is not changed by this local app action.

Timing

The current local cleanup and sign-out happen in the app. A practice-assisted account, privacy, portal-access, or medical-record request may require identity verification and additional time. Contact the practice for the scope, next steps, and timing that apply to your request.

Request deletion without app access

If you cannot use the in-app action, contact Julep Health by phone or email and ask for a Julep Health mobile app account-deletion request. Use this same contact route if you want to make a broader privacy, portal-access, or medical-record request. Please do not include medical details in ordinary email. The practice will explain how to verify your identity and complete the applicable request.

Questions and medical-record rights

An app-account deletion request is separate from a request to access, amend, restrict, or obtain an accounting of disclosures of protected health information.
