Thyroid dysfunction is one of the most underdiagnosed conditions in women — and one of the most misread on lab results. It's estimated that up to 60% of people with thyroid disease don't know they have it, and women are five to eight times more likely than men to develop a thyroid disorder over their lifetime.

The symptoms are easy to dismiss: fatigue, weight gain that won't budge despite effort, hair thinning, feeling cold when others don't, brain fog, constipation, low mood. Individually, each of these can be explained away. Together, they often point to a thyroid that isn't functioning at its best.

The TSH Problem

The most common disconnect we see is between what a TSH test shows and how a patient actually feels. TSH (thyroid-stimulating hormone) is the standard screening test, and its "normal" reference range is broad. A patient can have a TSH that falls within range while her free T3 — the active thyroid hormone that cells actually use — is low. She'll be told her thyroid is fine. But her body tells a different story.

Hashimoto's: The Frequent Miss

Hashimoto's thyroiditis is another frequent miss. It's an autoimmune condition and the leading cause of hypothyroidism in the US, but standard thyroid panels don't test for thyroid antibodies (TPO and TgAb), which are needed to identify it. A patient can have active Hashimoto's disease for years before it shows up on a TSH alone.
