First visit
A few things that help us get started.
Bring a current medication list, your insurance card if you plan to use it, and any outside records or recent labs that matter to the visit. If you are coming specifically for lab testing, we will tell you ahead of time if fasting or special timing is needed.
For personal medical questions, use the patient portal or call us. That keeps your information in the right place.
